Improvement Activity for Dispensing and Intervention Prescriptions of Powder Form Medicines

Abstract
If drugs with special dosage form are grinded, they lose the stability and the effectiveness.
We managed these drugs as prohibited powder form medicines. This study was conducted to improve of powder form dispensary system and establish guidelines. On average Kyung Hee University Hospital at Gang-dong issues 624 regular prescriptions of powder form medicines per day, and currently 4 pharmacists perform auditing, dispensing and inspecting of the prescription spending total of 8.5 hours a day. For improving effectiveness, we developed programs automatically block the input of powder form prescription of non-crushable medicines and notify the prescribing doctor of the reasons of such prohibition. Oral medications were categorized into ¡°12 groups of prohibiting crushing¡±and¡° 4 groups of unnecessary crushing¡±. Based on this study, out of 1,101 items of oral medications on the Center¡¯s drug list, 373 drugs (33.87%) were designated as non-crushable medicines. From April 2012 to June 2012, total 56,150 regular prescriptions were made for powder form drugs, 6,062 cases were determined to be unable-to-process which amounts to 10.79%. The most common basis for prohibiting crushing was enteric-coated. With the prescription software and operation enhancement, the unable-to-process decisions were reduced to 0 (0%) and the actual dispensing time for regular powder form drugs were reduced to 62 minutes a day. With this study, we could establish a guideline for the non-crushable medicines and improve the efficacy of the powder form medicine dispensary system. Department of pharmacy could reduce time of dispensing and intervention prescriptions of powder form drugs.
